Forrest was born on October 3, 1960 and passed away on Sunday, March 12, 2017.

Forrest was a resident of Smithfield, Virginia at the time of passing.

A celebration of life service will be held on Friday, March 17 at 11 a.m. at Trinity United Methodist Church, Smithfield, officiated by Rev. Jeff Cannon, followed by a reception in the church's Family Activity Center where the family will receive visitors. The reception will be followed by a private burial at Ivy Hill CemeteryCondolences may be registered on line at www.parrfuneralhome.com.
